Applications on prescribed format are invited on behalf of District Rural Health Society, Srinagar from eligible candidates who are permanent residents of District Srinagar only for contractual engagement to the below mentioned post under National Health Mission (NHM).

02	Lab Technician	Matric with Diploma in Lab. Assistant Training course from State Medical Faculty or any other recognized Institute.	5	–
03	Data Entry Operator	Graduate from any recognized university with six months diploma in computer application from any recognized inst. And having tying speed more than 35 words per minute.	4	–
04	Infant Young Child Feeding Practices Counselor	Bachelor’s degree in Dietics and Nutrition/ Home Science/Graduate with diploma in Nutrition and Health Education (DHNE) from recognized University.	1	–

TERMS AND CONDITIONS:

The initial contract will be for a period up to 31 /3/2016, extendable subject to performance review of the selected candidates and continuation of the scheme.
The selected candidates shall be paid monthly honorarium under National Rural Health Mission, as communicated by State Health Society J&K and contractual appointee shall not be entitled to any other allowances or monetary benefits whatsoever.

If at any stage even after joining place of posting, it is found that the selected candidate (s) have submitted invalid/ forged documents or misrepresented information; the candidate (s) shall be terminated from services.

Incomplete applications will not be entertained.

Application forms are available at Office of the Chief Medical Officer Srinagar on any working day from 10:30 am to 4:30 pm. The interested candidates may deposit their application forms personally in the Office of Chief Medical Officer, Srinagar or send their applications in the prescribed format within 5 working days from the publication of this advertisement notification. The Chief Medical Officer Srinagar shall not be responsible for any postal delays and application forms received after last date will not be entertained at all.
