District Health Society Jobs The applications should be addressed to the Chief Medical officer (Vice Chairman) of the District Health Society Rajouri on the prescribed format (Annexure-A). Interested candidates should apply on the format given as (Annexure-A), along with attested copies of supporting documents/certificates & Demand Draft of Rs.200/- payable in favor of Chief Medical Officer Rajouri, which must reach in the Office of the Chief Medical Officer Rajouri by hand against proper receipt within 21 days from date of publication of this notice.

How to apply:

Interested candidates should apply on the format given as (Annexure-A), along with attested copies of supporting documents/certificates & Demand Draft of Rs.200/- payable in favor of Chief Medical Officer Rajouri, which must reach in the Office of the Chief Medical Officer Rajouri by hand against proper receipt within 21 days from date of publication of this notice.

Initially the candidates shall be hired upto 31st March 2023 and further extendable on yearly basis upon the performance of the candidates/approval of the posts by the NHM

Domicile certificate issued by Tehsildar concerned.

2 Passport size Photographs.

Note:

No TA/DA shall be paid to the candidates for appearing in Screening test as well as interview.

Any application form found deficient in any respect shall be rejected without any intimation.
