Applications are invited from eligible candidates for contractual hiring of services under National Health Mission in District Shopian for following vacant posts as per details given as under.

Remuneration Rs. 12600.00 per month (Female Candidate Only)

Matric with Diploma in FMPHW Training Course from SMF OR Any Other Recognized Institute. Age: up to 45 Yrs.

a.              Screening Test = 60 Points

b.              Matric = 10 Points

c.              Diploma in FMPHW -15 Points

d.              Viva Voce = 15 Points

1= RBSK Team 2 = Subcenter Tulran

Important Instructions

1 The candidate should be permanent resident of District Shopian.

2. The hiring will be purely on contractual basis. Initial contract for hiring shall be for 11 months or 31^v March 2017, whichever is earlier, and further extension will be subject to satisfactory performance of the candidate / approval of the post by Gol.
3. The interested eligible candidates are requested to submit the application forms complete in all respects in the office of Chief Medical Officer Shopian on or before 25′^h 2016 by 04:00 pm.
4. The candidates who have obtained degrees through Distance Education Mode from the Universities recognized by the UGC as mentioned in Govt. Order No. 252-HE of 2012 dated: 30-05-2012 issued by the Higher Education Deptt., Civil Secretariat. J&K after 2005 shall have to submit a certificate from the SMF along with application form stating that candidate had undergone the degree from the University head quarters are recognized with the Govt. Of J&K/ SMF. Application shall not be accepted in absence of this certificate.
5. The candidates who have obtained degrees/diplomas from universities outside state have to submit a certificate from SMF along with the application form stating that the university is recognized for the period the degree/diploma has been obtained.
6. In case of deficiency with reference to the eligibility etc proved at any stage of time the candidate shall forfeit their selection.
7. In case at any time it is found that any candidate has used any fraudulent means or certificate (PRC, Marital Status) to avail local criteria, his / her selection will be cancelled.
8. The number of vacancies is only indicative and may increase or decrease.

List of documents to be attached with the application form:

• Date of Birth Certificate
• Permanent Residence Certificate
• Degree/ Diploma from the recognized university/ institute for the required qualification as per the post applied for.
• Marks sheet of all semesters /years and one consolidated marks sheet indicating marks of all the semesters/ years of the required qualification for the post applied for.
• Certificate from the concerned University along with application form stating that candidate had undergone the degree from the University head quarters and not from the off-campuses which have been established by these Universities beyond their territorial jurisdiction.
• Experience certificate in relevant field with date and stamp of the issuing authority.
• Applications not falling in the prescribed criteria or without aforesaid documents shall be rejected out
