Application on the prescribed form are invited from the Urban poor living in the jurisdiction of Jammu Municipal Corporation, Municipal Committee Samba, Bari Brahmna & Akhnoor.

Interested candidates of Jammu Municipal Corporation are requested to collect application form from the office of Chief Executive Officer, Jammu Urban Development Agency, Dr. B.R. Ambedkar Chowk, Narwal Road near Petrol Pump, Jammu and candidate residing in Municipal Committees may collect from the office of the concerned Executive Officer during office hours and submit the same by or before Last date along with following formalities.

1. Income certificate (from Revenue Department having annual family income from all sources upto Rs. 1 Iakh)
2. Two photographs
3. State Subject
4. Identity proof
5. Affidavit duly attested by notary.
6. Date of birth certificate.
